A Representation Agreement is a replacement for what used to be known as a "Living Will". Generally, it provides for the appointment of (1) a Representative, (2) an Alternate Representative and (3) a Monitor to oversee the actions of the Representative or Alternate Representative. These appointments are made for the purpose of looking after all of the interests of the person making the appointments, both financial and health care related, including refusing life support measures, in the event of future illness or incapacity of that person.
As this form of Representation Agreement gives the Representative, or Alternate Representative, the maximum powers allowed by the Representation Agreement Act , the document must be signed in the presence of a Lawyer or a Notary Public.
The package offered includes Explanatory Notes and Signing Instructions as well as all Forms required to be signed by the Representative, the Alternate Representative, the Monitor and the Lawyer or the Notary Public.
