The forms are listed below for your selection. GENERAL authorizes everything, SPECIAL authorizes only specified things and ENDURING remains valid after your subsequent mental infirmity. Each form comes with Important Information and Instructions for completing the form. Please be sure to read these instructions before you start filling in your Power of Attorney or related Form.

While our forms cover an array of different situations, there are scenarios where a Power of Attorney form is not appropriate and the advice of a lawyer or a notary public should be sought for the preparation of your Power of Attorney or other legal document. Some examples of scenarios when you should seek such advice include:

you do not find the forms provided sufficiently cover the objectives you are seeking to achieve in your individual situation;
you are an older person, if there is any question about your mental capacity, or if there is a possibility that you are being unduly influenced by a person or persons who are trying to induce you to sign the Power of Attorney; or
you have a large and/or complex estate or the matter you are dealing with is large and/or complex.

Power of Attorney Form 12	Revocation of a Power of Attorney for an Individual (not for a Corporation). Also includes a Notice of Revocation of Power of Attorney form required by the Land Title Office.	$17.99
Power of Attorney Form 13	Revocation of a Power of Attorney for a Corporation (not for an Individual). Also includes a Notice of Revocation of Power of Attorney form required by the Land Title Office and a Resolution of the Directors of the Corporation revoking the Power of Attorney.	$17.99
Power of Attorney Form 14	General Power of Attorney for the appointment of one Attorney. A special provision has been added so that the Power of Attorney will not expire after 3 years after the date of execution.	$17.99
Power of Attorney Form 15	General Power of Attorney for the appointment of one Attorney. A special provision has been added to authorize and empower the Attorney to use the Power of Attorney to sell, transfer or charge to or in favour of him/herself any land owned by you without restriction. This is not ordinarily permitted to be done by an Attorney without this special provision.	$17.99
